After completing a family analysis and assessment with various models, I would like to reflect on how I can better utilize these tools in my future and gain a better understanding of family centered care. I will be using the Rolfe, Freshwater, & Jasper (2001) model to complete my reflection and weigh the pros and cons of assessing the family with the Calgary Family Assessment Model, the genogram, and the ecomap.
I started making the genogram first, this was probably the most laborious part of the assignment. Getting used to the computer programming took the majority of the time. The genogram and the ecomap positively impacted my new perspective on the family. They provided an organized display of the family patterns and vulnerabilities. The genogram contributed to a more detailed representation of the family structure and the relationships within the family. The ecomap contrasted this by providing a resource to observe the external factors that cause stress or support for the family. The ecomap can be compared to the social determinants of health and the domino effect that one social determinant of health can have on the others (Kozier, et al.). Just like a negative social determinant of health, the stressors in the ecomap influence and produce other stressors. I think the ecomap illustrates a clear path towards reducing the stressors in the family and identifying the source of the problems. These tools helped me learn the importance of a comprehensive family assessment. It would have been difficult to link the relationship between the stressors and family patterns had it not been for the genogram and the ecomap. I think that this will give me a more objective point of view. However, I will bring this experience forward into my nursing career. Family dynamics are worth understanding whether the whole family is being cared for or not because the family plays a major role in patient care and support. Therefore, it is crucial to know as much about the family dynamics as possible. I think that the ecomap will also be a quick and practical tool that I can use to determine the best discharge care planning for a patient. Understanding the struggles in their environment, and which stressors need to be dealt with first is very important to the client's ability to function at home. Although I can't picture a situation where I might use the genogram explicitly in that format, I do believe that knowing the relationships that your patient has with their family is very important. If there is someone who directs negative energy or is hostile to your patient, then chances are the patient doesn’t want to see them. A pro of the map would be, that could be helpful for a client to view the map and see where they fall within the scale. Balanced couples/families will function more adequately than Unbalanced couples/families (Olson, 1999). This map would be beneficial if, for instance, the disengaged / rigid person views this tool and realizes that becoming more flexible connected would be best for their children and marriage. Or the opposite if a mother was chaotically disengaged this system could help her to have the awareness of how this behavior is effecting her family negatively. Another pro would be when this is used as a marriage theory tool it would be helpful for each spouse to view where they fall on the map and if that has a negative effect.
Genograms are a great way to organize different subtypes of your family. You can organize a genogram with generations, authority, power, and age. When I organized my family in these groups I realized that age sometimes does not correlate to how much power the family member may have. I thought I knew everything about my family members and their roles but, after doing this assignment it made me make better conclusions why some people have more say than other do in the family. Genograms also helps make connections why you are not as close to some family members but closer to others.
Family Assessment Tools The Calgary Family Assessment Model (CFAM) is a renowned assessment tool used by nurses in conceptualizing and organizing data gathered while working with families(Wright and Leahey, 2009). It is a useful tool both for compiling data for a family assessment as well as in assisting families to deal with specific health issues. The CFAM has three main categories namely: structural which encompasses the internal, external and contextual; developmental which includes stages, tasks and attachments; and functional level which hasthe instrumental (daily living activities) and expressive which includes communication, problem solving, roles, power, beliefs, alliances and coalition(John & Flowers, 2009).

The internal aspect focused on the family composition, order of birth, and boundaries; the external aspect included the extended family (Leahey & Wright, 2016). Context included the broader aspect of patient care, which included their race, ethnicity, social class, religion, and environment (Leahey & Wright, 2016). Development within the CFAM included stages of development and attachments of family members to each other. The functional part of the CFAM included activities of daily living, as well as expressive components which were comprised of communication, problem solving, and roles (Leahey & Wright, 2016).
The information presented, even though very informative, lacked sequence. Vaughn starts off first by listing and elaborating on the impact of the change in family structure, then goes on to describe family functioning (which takes in the description of family structure), then back again to the impact; this should have not been so. Being that the writer uses the cause and effect method of organization to convey her message to the readers, then she should have begun with the description of Family functioning, then continue with the consequences that the change in family structure has on children. Structuring it that way would have made it easier to understand. The first paragraph under the heading “Family functioning would have been a more suitable introduction to the article, accompanied by the thesis outlined in the beginning sentence of the article, also “the Impact of Parenting Stress” should be the title for the last paragraph as the focus is really on parenting stress.
