Ms. Punzo alleges that Mr. Taylor, during the July 28, 2015 safety meeting, directed a belittling comment at her to the effect of, “you don’t lift a finger to help out around here.” After interviewing all witnesses it is evident a comment was made towards Ms. Punzo. Three of six witnesses stated they directly recall Mr. Taylor making a comment. When asked for context for the comments, the overarching theme from all the witnesses was that it was most likely due to tensions from the recent facility remodel or a sign storage project that Mr. Taylor and Mr. Page were responsible for. Punzo alleges that Mr. Taylor on an unknown date in July 2015 was aggressively slamming his wall locker and wall locker door. She doesn’t state if there were witnesses present or not or as well when the incident occurred. After interviewing all witnesses, and by Mr. Taylor’s admission, it is evident that an incident occurred. However, there is no clear evidence that the wall locker and wall locker door were aggressively slammed or that it was clearly directed towards Ms. Punzo. Four of six witnesses reported that they either saw the incident or heard it from where they were at during that time. Only one witness describes the incident as Mr. Taylor throwing a “baby fit,” the others all say they heard the wall locker door being slammed. Only one witness believes he heard an exchange between Mr. Taylor and Ms. Punzo. He couldn’t recall the exact conversation but he believes Mr. Taylor might have made comments about needing room for his gear and that Ms. Punzo might have been raising her voice. Mr. Taylor openly admitted that an incident occurred; however, he provided context behind the incident. After the remodel of the crew room, the wall lockers were in a new configuration. Punzo alleges that Mr. Taylor has made repeated comments about the women’s bathroom, with one specifically of, “Why do we have to have bathroom locked for just one woman – not fair.” Ms. Punzo doesn’t provide a date that this specific comment was made; she just states comments, such as this one, have been made on multiple occasions. The context behind this complaint dates back as many as 6-7 years, per a witness statement, and the issue doesn’t exist now with the completion of the crew room remodel. Prior to the remodel, there was one bathroom for male and one bathroom for female. The male bathroom only had one toilet, as was the case for the female’s bathroom. The female bathroom was locked and only Ms. Punzo and Mr. Smith had the key. More than one witness stated that male employees were only given the key to the female bathroom on rare occasions. This arrangement had caused male employees to have to wait in line to use the bathroom or, per one witness, go outside to relieve themselves. Per witnesses, over the preceding years leading up to the crew room remodel the topic of the bathroom has come up many times and by multiple people. With the crew room remodel complete the male restroom now has multiple toilets and the female bathroom is again locked. On 7/20/2015 S/O EMT Perez was dispatched to MG-601 regarding a fall. S/O EMT Perez knocked and announced his presence at the door, when there was no answer S/ O EMT Perez annonced his presnece again and began to search the apartment. S/O EMT Perez found the resident, a Mrs. Doris Cotts on her bathroom floor by her toilet. Mrs.Cotts stated that she had slipped from her motorized scooter and fell onto the floor.
On 9/25/2015 SO EMT Perez was dispatched to VP-113 regaurding a fall. S/O EMT Perez knocked and announced his presence at the door and was greeted by the residents spouse and introduced to the fallen female resident a, Mrs. Dorothy Gish who was sitting upright on the living room floor by her rollater. Mrs. Dorothy Gish stated that she was sitting on her rollator when she slid onto the floor and was unable to pick herself up on her own strength. S/O EMT Perez performed a pelvic exam on Mrs. Gish which revealed no pain or grimace.

Will Smith has been a victim of second degree murder by a man named Cardell Hayes on April 9th 2016. It started off as a simple hit and run on Mr. Hayes which slowly but surely escalated to Hayes pulling out a gun and fatally shooting and killing Mr. Smith. On a surveillance video it shows a Mercedes SUV trailing a Hummer, until the latter vehicle stops abruptly. The Mercedes pulls up quickly, too, possibly hitting it from behind. On the night of December 28th, 2009, Erin Lee MacDonald had guests over to his condominium in Halifax. The concierge, a Mister Sears, of the building received complaints because MacDonald was playing music very loud. Sears responded to them by visiting MacDonald at his unit and asking him to turn the music down. MacDonald responded aggressively, swearing at him and saying he would not turn the music down.
