Develop the concept/idea/issue you selected from your practice area into a relevant question This paper will evaluate and address issues surrounding never event in healthcare. In this regard, the guiding question for this analysis is, ‘How can never events be prevented in acute healthcare institutions?’ Describe the question According to Wilson and Giddens (2014), never events are adverse situations and serious medical errors that a patient should never experience. For instance, wrong medication, wrong surgical site, administering contaminated drugs to patient or sexual abuse of a patient within a health care facility. In most instances, these events are preventable but upon their occurrence, they are costly, both financially and reputation-wise to the affected healthcare institutions and the patient. Therefore, never events can be prevented by finding out the source of error or the near misses and developing mechanisms to prevent these events from occurring. Working through the four steps of the data, information, knowledge, wisdom continuum Moen and …show more content…
As a result, this will provide the knowledge of the rate of incidence and understand the conditions which were involved for such events to take place in order to initiate proper prevention measures from their taking place again. Can informatics be used to gain wisdom? According to Ball, Douglas, and Walker (2013), informatics is the integration of nursing practice to information technology in order to promote health. As such, informatics can be used to gain wisdom. In this regard, understanding the information stored electronically, forming knowledge based on information gathered from health record concerning particular issue such as never events will provide essential wisdom over a period of time. In addition, it will assist in placing the case under investigation in context. Describe how you would
Assignment – There are five common purposes for medical records. List each of these purposes and provide an example of each in healthcare. Having good medical records is very important, for the proper care of patients. “Medical records can be used to manage healthcare, track healthcare, provide clinical data, meet regulatory requirements, and document healthcare” (Allen, 2013, P. 57). Without the proper documentation there is no proof that it was ever done.
Kalb explains in order to reduce risks, doctors must learn skills to safety procedure and teamwork techniques from the beginning of their education, having chance to interact with real life situations with team members by using their technical skills. At first, the title of the article Do No Harm: Medical Errors Kill Some 100,00 Americans every year. How we can reverse the trend is straightforward for the reader to effortlessly understand what the author is going to say. The way that medical treatments were somewhat supposed to be safe, has killed hundreds and thousands of Americans every year even
Hence, this is a sentimental event because this unanticipated event resulted in death to a patient, not related to the natural source of the patient's illness. Therefore, the threat and error management model should be used to determine both training needs and organizational strategies to improve the management of threats to safety. What defenses in the system failed in this case? Can you construct a Swiss cheese analysis of the system defenses and what occurred?
Interestingly, the findings from the review of literature shed light to the challenges nurses encounter with the implementation of electronic health records and identify areas for improvement that could be made in an effort to achieve the goals of the HITECH act. Based on the review of literature, overall, the electronic health record is seen as a positive aspect to assisting nurses in providing positive outcomes for patients. However, challenges still exist with the daily utilization of the EHR, with communication among healthcare providers and interdisciplinary teams. These challenges present nurses with great difficulty as they attempt to provide care to their patients. Because some nurses continue to struggle with utilizing the electronic
The Institute of Medicine (IOM) published a report in 2000 that estimated there were around 100,000 deaths each year in American hospitals from medical errors. IOM results were mostly based on errors of comission. In ICUs, the errors of omission are much larger as compared to the errors of commission. The number of patients dying becomes even higher if these errors are included. The follow-up report by IOM in 2001, provided a direction towards the need for making the basic changes in the health care delivery.
The Date-Information-Knowledge-Wisdom model is a model that is essentially aimed at providing the nursing profession with a framework to support the practice of nursing and the delivery of nursing care (Ronquillo, 2016). This framework encompasses the idea that data, information, knowledge, and wisdom can be useful in the world of nursing informatics, and these four aspects can act as a guide to identify solutions to many nursing problems (Ronquillo, 2016). When dissecting the model into four parts, each component contains a different meaning, or use. Data means concrete information that has not been interpreted yet, information means organizing or interpreting the data, knowledge means relationships are formed from the data itself, and wisdom
Obtaining certification in a specialty area is one way a nurse confirms she has achieved the necessary knowledge and skill within that practice area to be considered above average (Rauen, Shumate, & Gendron-Trainer, 2016). In order to realize my goal of board certification as a master’s prepared informatics nurse, I must first gain the required education, expertise, and competence through classes and practice experiences. This paper will discuss how each MSN core course, each MSN specialty course, and each potential practice experience will improve upon the competencies needed to obtain board certification in informatics after obtaining my MSN from American Sentinel University. MSN Core Courses MSN Role Development (N501PE)
There were specific situations that led to the cause of Julie Thao's actions of medication error and the death of Jasmine. The situation could have completely been avoided had Julie followed the code of ethics and avoided shorts to provide proper care for the patient. The state claimed that Thao's mistake was caused by actions, omissions and unapproved shortcuts, however, there were other factors that played a role in her carelessness as well. While failure to comply with procedure has been a factor in the medication administration error, other factors contributed as well. For example, failure to properly use the information system, or to ignore alerts or warnings have also resulted in preventable errors (Nelson, Evan, & Gardener, 2005).
Social media platforms continue to change the healthcare landscape by allowing for users to see a variety of information and points of view. Social media can be used for information and education, marketing, recruiting, and news updates. Healthcare organizations and consumers can utilize social media to improve health and knowledge. Social media can also be used for research, disease tracking, and to monitor trends. This paper discusses the controversial public health topic of abortion and what people are saying regarding it on Twitter.
I will summarize each outcome for the Nursing Informatics specialty. For the intent of this paper I will use outcome and competency interchangeably. The first outcome means the ability to gather healthcare information across the continuum of care; combine and utilize the information gathered to develop a process. Finally execution of that process to evaluate its ability to improve the quality of the healthcare environment. Healthcare managers are constantly assessing patients and collecting information.
Unfortunately, human error is a large contributor to patient death, and these actions can be avoided if interprofessionals were to collaborate
Technology is used in the health care system to chart patient health information, communicate with the interprofessional team, and monitor plan of care. The nurse, upon licensure, recognizes the importance of computer literacy and the effect technology has on patient outcomes. The use of technology can hinder or help the patient based on its use. For this reason,
Adverse and near-miss events occur in the real-world of nursing practice. The difference between an adverse event and a near miss is the outcomes of the event. Adverse events cause harm to the patient through medical care and may be through negligence, which can be preventable (Agency for Healthcare Research and Quality, 2016). A near miss is an event that might reach the patient but does not cause harm to the patient.
emplate Observations (Similarities/Differences) Ethics My future role is Family Nurse Practitioner. Ethics deals with the actions of being right and wrong and what is good and what is bad (Barker & Denisco, 2016). The ethical guidelines provide advanced practice nurses their job description within the scope of practice and prevent them from underdoing or overdoing their job. My comparison role is as Graduate Nurse.
That’s just my personal opinion. Living and working in the digital age brings me to my second choice of Informatics and Technology. The Massachusetts Department of Higher Education (2010) explains the importance and “necessity for all health professionals to seek lifelong, continuous learning of information technology skills” (p. 22). Electronic Medical record systems are the future. Soon there will be no paper.
