As I travel North America teaching our Medical Exercise Specialist workshops, I have noticed the highly successful Post Rehab Professionals (PRP's) demonstrate eight habits that guarantee their success. We will discuss each of these 8 habits and explain the impact each has on a post rehab practice. Successful PRP's are able to develop safe and effective post rehab programs for a wide range of medical conditions, establish long-term referral relationships with medical professionals, and receive insurance reimbursement for post rehab services. If you embrace these habits, I guarantee you too will grow a profitable and successful post rehab practice. PRPs present their scope of practice and role in a clear and concise way in their brochures, marketing materials and presentations. Presenting your scope of practice and clearly specifying your menu of services allows you to market yourself as the expert in the rehab spectrum, rather than relying on an outside individual who lacks a the necessary understanding of your background and education. Relying on someone else could result in a wrong image that could negatively impact the direction of your practice. Dimock center is a nonprofit and government founded organization, which means the staffs are very little compensated for the relentless work they do. This survey helped me have a better understanding of inpatient and outpatient intensive rehab treatment. This organization believe in giving this special population many chances to rehabilitate and transform their lives for better outcomes. After treatment, patients are still followed by their designated case worker.

I constantly stay ‘plugged into’ my recovery community in a variety of different ways. I know that it plays a huge part in my personal recovery in helping me stay clean and sober. A few of the ways I am able to stay connected are through hospitals and institutions, as well as picking up service commitments at my home group. I am a big advocate of H&I’s because they were a key component of my recovery when I was in treatment. I was tremendously inspired when I heard someone who was doing well in the outside world share their experience, strength, and hope.

Within the MFP (Money Follows the Person) program, there has been numerous research performed regarding reinstitution rates, and smaller focus on transition strategies to adequately prepare consumers for returning to the community. Current practice focuses on initiatives to transition persons in institutional settings to community-based waiver services; individuals do not have to wait for community-based services, which have long interest lists. The policies in place outline best practice of collaboration across the broad spectrum, however there is no outlined focus on preparation for success. It is the goal of this writer to develop a standardized plan of care, utilizing evidence based practice to improve patient outcomes. This quote illustrates the feelings the drug addicts get from communicating with people who have been through similar situations. The people with addictions connect with the workers at the Insite and feel comfortable sharing their stories with them. To enhance my experience I would like to work with veterans, adult homeless, and the mentally ill. My objective is to be admitted into the Master of Science degree program with an option in Rehabilitation Counseling, where I can grow and take on new challenges. Ultimately, I would like to assume further educational and gain work experience that would allow me to work more effectively with people with disabilities. Most importantly, I would like to work for an agency where I can build a career in counseling and possibly management. I am driven to be the best at what I do.
As I progress in my career, I feel that it is important to develop a personal style and technique that clients can relate to. I am also interested in learning more about assessments and evaluations as they drive the rehabilitation plan. While I have assessed hundreds of clients, I realize that there is more to learn. For instance, the trend over the next five years will be an increase in applicants who receive state-plan home and community based services. The Final Rule issued by The Center for Medicare and Medicaid will require that individuals receive services in the most integrated settings.
