Some of the key problems of handoff arise from disintegration of EGRs (Electronically Generated Records) during handoff and lack of structured formats for handoff communication. Despite the introduction of electronic health records, nurses continue to use verbal reports and non-digital reports to relay information during the handoff process including the use of scrap papers. One can easily find out that there are no standardized handoff procedures in a number of healthcare facilities across the globe. The lack of a standardized handoff procedure at this facility has been supported by a study by Alghenaimi (2012). Of the 20 nurses who were interviewed by the researcher, none reported the existent of a standard written handoff policy, with 71.35% of the 112 nurses from the hospital reporting that there was no actual handoff policy at the facility. Therefore, it can be generally stated that the current handoff procedures hardly succeed to meet the required international standards of handoff

In studying Registered Nursing one will find that they have to be compassionate and want to care for others. Registered Nurses assist physicians with suffering patients of any medical condition. They help to decide the patient’s medicine, treatment, recovery, and educate the them and their families on post-medical treatment. They need to have bedside manner, and be able to prepare patients for surgery. Registered Nursing careers range anywhere from critical care, oncology, mental health, to an everyday school nurse.
My capstone project is about the need for nurses to learn about cultural diversity, cultural awareness and cultural sensitivity. This has been a fascinating project to explore and I am learning about so many cultural beliefs. When a patient's cultural practice is understood competent patient care is rendered and health care disparities are eliminated. For example, in some cultures many people believe that illness and evil can be prevented by following strict religious codes, morals and practices.

Glen (1998) also discussed the importance of having a structured handover process stating that it will lead to an development in the quality of care delivered (K. Chung, 2011). The literature review reveals bedside theme emerged from nursing handover. The current research available may not be substantial but it does indicate support from the large amount of anecdotal evidence which claims that nursing bedside handover is an effective form of handover process.
With the use of consistent handoff tools, there is likely not a missing piece of information that leaves the receiving nurse with gaps in any information. The inconsistency of use of different tools such as SBAR (Situation-Background-Assessment-Recommendation) or electronic handoff methods is where information gets lost. The use of SBAR is commonly used to maintain uniform communication. In example of the use of SBAR against the situation mention earlier, the known history of imprisonment with bloody sputum were not mentioned in the background or assessment piece of handoff. Not only did this impact patient safety but also the safety of nursing staff.
Proper communication and interaction are some of the most fundamental elements of successful nursing. It is very easy to occur a miscommunication when not all the healthcare providers are in same pace. Miscommunication can occur during end off shift report among the nurses. Some of the factors of miscommunication includes; it is a busy time when nurses are giving report, they are tired, wants to make shortcut, and language barrier. Furthermore, nurses that are trained in foreign country have difference in nursing practice.
Not all heroes wear capes, they wear scrubs too. The career I plan to pursue is a nurse practitioner. A nurse practitioner diagnoses and treats illnesses, independently or as part of a healthcare team, such as hospital. Their focus is disease prevention towards their patients and everyone. Nurse practitioners may order, perform, or interpret diagnostic tests such as lab work and X-rays, they may also prescribe medication.
Bedside shift reporting is used in many health care facilities to promote a beneficial handoff for both patients and nurses. This type of reporting is an important process in clinical nursing practice because it allows staff to exchange necessary patient information to guarantee continuity of care and patient safety. “Moving the change-of-shift handoff to the patient’s bedside allows the oncoming nurse to visualize the patient as well as ask questions of the previous nurse and the patient” (Maxon, Derby, Wrobleski, & Foss, 2012). I am currently an undergraduate at Rowan College at Gloucester County completing my pre-nursing studies. As I was growing up, I always enjoyed the idea of helping and healing others. My inspiration to become a nurse was watching my mother make a difference in the lives of elderly at a local senior living community as an Activities Assistant for years. Even though, she was unable to go back to school due to personal circumstances, the compassion she showed towards others around her was something special as she took care of them as if they were relatives. Whenever I had free time, I would just sit and play a game with the elderly residents hearing their life stories from when they were young.
In creating this portfolio, I am inclined to maintain a professional e- handbook, so to speak, of my educational and professional accomplishments over the years that reflect growth and development in my quest to be a competent nurse. Creating an e-portfolio allows for the safe documentation and archiving of my information that potential employers can have easy access to when I am in the process of seeking employment opportunities. Having an e-portfolio is a more efficient and effective way of utilizing technology to be more organized in keeping track of important information that could otherwise get lost or destroyed due to adverse events such as floods or fires.
