On 11/4/15 Ms. Brittany Terry and her daughter Lundyn met with CM for an ILP. CM introduce herself as the client temporary CM assigned to her case. Housing: Client 's exit strategy is to move by 2/18/16. Client must maintain employment and submit pay stubs weekly .Client is expected to comply with savings and have saved enough for one month 's rent and security by 12/18/16, Client is to seek for housing and obtain housing by 1/18/16.Client was reminded she must meet with her Housing Specialist Mr. Brown on a bi-weekly basis, comply with all housing requirements and explore all available housing options.
Social Service: Client must meet bi-weekly with Case Manager for ILP Review appointments. Client was reminded she must call in advance to reschedule her appointment if she is unable to attend. She is currently scheduled to meet every other Wednesday.

CCIB received a SOC 341 from APS for the following residents: Daniel Hourihan (DOB: 10/03/54), Gerardo Guerrero (DOB: 01/02/95) and Gerald Gaither (DOB: 06/14/59). Per the reporting party, Lee Nln, the provider/Owner Cheryl Oliver has been advertising her independent living facility as a board and care. RP stated that her clients have been paying the board and care rate and the client require medication management. RP stated that in April 2018, client Daniel Hourihan moved to another facility. The rent for Daniel was sent to Ms. Oliver who continued to cash the rent checks for Daniel.
I believe everyone on this email thread was aware of my meeting today with Joe Baldwin, Guardian, of Kathy Rennich to discuss her recent return from inpatient rehabilitation at a local nursing facility and her expressed desire to move to the Hensgen Home. Basically, in February 2017, Kathy fell resulting in a fractured tail bone. She received inpatient rehabilitation at Care Springs for fourteen days and has returned home with PT services. Since her return home, Kathy has refused to participate in ADL’s (which isn’t a change in pattern as she refused prior to the nf stay) and is demanding that she have the opportunity to live at the Hensgen Home. Kathy’s reasoning behind wanting to move the Hengsen Home isn’t exactly clear to the team.
On 09/29/2015 at 3:18 PM SC received call from Marguerite Pa’s niece who reported that Pa has identified two girls Gabby and Yajaira he wants to be his aide but the agency told them his service were terminated. SC attempted to explain why the service was interrupted and next step; SC end call with Marguerite. SC then called Vital Support and briefly explained to receptionist and requested to speak with a manager. SC was transferred to Vitaliya at Vital Support Nurse manger. Vitaliya stated that the termination date is incorrect and service was provided to Pa beyond termination date and that SC need to change the dates so that they can receive payment.

worked for the Children’s Center of Cicero Berwyn as a part-time Home visitor for six months from September 2016 thru March 3rd, 2017. She was a good employee who had passion to work with children and families. She created lesson plans for five children and families, prepared developmentally appropriate activities, observed children, provided resources and referrals to families in the communities. Her doctor had instructed accommodations and those were in place for her. Those accommodations included not to sit or stand for prolonged periods of time and not to lift more than 10 pounds.

The referrals were not received or processed, however you completed a Case Management Visit on 4/1/16 regarding information contained on the referrals. How would you like me to handle the referrals. I can attach a LIC812 on your LIC809 if you prefer. I don’t believe I need to process a complaint due to your case management visit.

Did Ms. Miller receive other disciplinary actions, i.e. written reprimand, suspensions, etc?

Your description of Seven Counties Services as an organization that aides individuals “who are fighting addictions, abuse, mental illness, and developmental disabilities” is remarkable (Mrock, 2016, para.1). What a wonderful organization. Crutchfield and Grant (2012) stated that “high-impact nonprofits engage in both direct service and advocacy”(p.49). It is clear from your post that Seven Counties Services provide a service to the individuals whom it engages.
Mrs. Navarro asked the CP if the CP can provide her with information about entering the shelter, at which the CP agreed to do so. Mentoring Program: The CP provided Mrs. Navarro with information about SCR and mentoring programs for Tyler to explore. Mrs. Navarro reported that she enrolled Tyler and Sidney for the PAL afterschool program, but Tyler refuses to attend-so, the child’s application is placed on hold, according to Mrs. Navarro. Tyler was questioned as to why he is not going to the after school program and based on the child response, he could not provide a proper answer.
i.10The sponsor is providing the minor with a safe environment. The placement is located in an apartment complex in Woodbridge, VA. The placement appears to be safe and no visible hazards were observed. The minor is provided her own bed and shares the bedroom with other sisters. The minor did not report any concerns regarding her safety, the placement or the community.
August 14, 2015 FTF with Ms. Cheryl Whited by Caseworker Cheryl Pellette Ms. Whited stated Ms. Bradford and Katie will be living at the residence. She has helped Ms. Bradford on the years. She has noticed Ms. Bradford change of focus. Ms. Whited stated Ms. Bradford is in the process of terminating her parental rights to Skyla and Mia. Ms. Whited stated Ms. Bradford shouldn't be around Mr. Clawson or her other friends which are a bad influence.
Informing the client about available options regarding her health care, (Army) benefits, Medicare, food stamps, and how she is utilizing community resources. I would encourage that Mrs. Carla Washburn stay in contact with friends and family members to form supports that can assist her. She has very strong family values and spiritual guidance from her traditions. It is recommended that she find a home health provider or companion care resource that helps her with household tasks, transportation to doctor’s appointments, grocery stores, and church activities. Have a visiting nurse check in once a week to check how she is doing.

The agency employees consist of over 20 staff that has many years of experiences in health care, home care, and basic needs field. Metro Social Services is a free service agency open to the communities near Birchville, California. Identifying Information: Ms. Joan Cassell is a 23-year-old young Caucasian woman who lives at home with her family.
Once Hazel entered the program, she begin working with DHR and Secure Dwellings to develop a plan for her to get her child back. Hazel’s son is now back with her and his youngest brother, a family reunited because of the stability the program afford her. We are currently assisting Hazel and DHR with getting Hazel permanent housing and employment in Birmingham. If everything continues to progress, Hazel will be in her own place by or before the date of this board meeting.
Ms. Stevens and her child, Tjayda Stevens are receiving preventive services with Harlem Dowling Westside Center for Children and Family Services (HDWC). Ms. Stevens’ case was referred from ACS and accepted thru HDWC on 03/02/2015. The services provided to Ms. Stevens are casework counseling, community advocacy, housing advocacy, and monthly home based visits to ensure stable family functioning and child safety. Ms. Stevens has been receptive to services since the agency obtained case planning responsibility. Administration for Children’s Services recommended the following service plan for Ms. Stevens to complete parenting and anger management class, individual counseling, and domestic violence.
