At this time the client presented with a readiness to change and is highly motivated to receive treatment at MRFH. The client was diagnosed with Alcohol Use Disorder: Severe, and Cocaine Use Disorder: Severe. The client stated, "I am pretty much homeless and I left the Salvation Army after I was about to get into the Step Up Program. There were so many drugs at the Salvation Army. I want to learn more about myself and my addiction. I removed myself from my job so I could receive treatment at MRFH, and be around other people who are trying to recover. I want to work on my spiritual condition. I went from being the manger of an Oxford House, to using drugs and alcohol, eventually becoming homeless." The client has one 16-year-old son that he shares joint custody with his wife. Female and male cousins growing up, touched him and his brother inappropriately, and made them perform oral sex, he was around five-years-old. His cousins father was sexually molesting his cousins, and once the clients father found out his sons were being sexually abused he advocated for them. The client stated he feels as though he was neglected by his mother because she would often leave him and his brothers with his father who was physically abusive so she could drink alcohol. Prior to participating in MRFH the client was managing an Oxford House. The client has been arrested four times, and has been sentenced to prison once. The client stated he had an addiction related to shoplifting at once. The client stated he shoplifted to buy drugs and alcohol for his wife and himself, and he wanted to be the best shoplifter that he could be during his active addiction. The client last two jobs ended with him seeking treatment services for substance abuse. The client reported that he has no medical or food allergies. Co-occurring disorders are common with most client cases that are presenting with a substance use disorder. Rosa is presenting with a history of several suicide attempts, alcohol use disorder, Post traumatic Stress Disorder (PTSD), and Borderline Personality Disorder (BPD). The client’s most severe symptoms are anger, fear, and shame. It is these symptoms that are complicating her life, causing distress, and self harming behaviors. Additionally, her treatment history is limited since she does not finish her therapy sessions.

Identify the client Allen Mack is a 40-year-old African American male currently serving six months of the remainder of his 10-year sentence at the Cherry Health-East Half-Way House in Detroit, MI. Mr. Mack was indicted for Drug Trafficking/Conspiracy and enrolled in the RDAP (Residential Drug and Alcohol Program) program while in prison. RDAP is the Bureau’s most intensive treatment program. CBT is used in a modified therapeutic community model where offenders experience living in a pro-social community.
Pt has history of drug abuse and numerous admission to substance abuse program for heroine and crack/cocaine abuse. She was able to maintain abstinence for certain period of times only during incarceration. Pt has recently arrested after fighting with a woman, whom pt perceived as threatening. Pt was arrested since pt's attack led to the other woman's injury. Pt is currently on probation for 4 1/2 years for second degree assault and theft.
The mother has four priors the two prior adult intakes with physical injury by Paramour. She was listed as a victim with mild retardation. Shaquana states at the time of the two adult priors in 2006 and 2009 the play fighting she did with the paramour at that time sometimes have gotten a little rough. Another prior the mother was a HM during family violence investigation in 2012. One last prior she was the AP with no indicators in environmental hazards, substance misuse alcohol, and substance misuse-illicit drugs.
Patty should answer to and resolve all pending legal issues and adhere to the decisions of the courts responsibly. A psychological evaluation needs to be conducted to determine Patty’s cognitive functioning relating to intellect, vulnerability and personality. The evaluation should also assess life events that contributed to substance use. Treatment recommendations will all be dependent upon the resolution of current legal issues. Patty has never undergone any treatment for substance dependence; therefore, intensive treatment for identified addiction to opiates, alcohol and other illicit substances should be a primary goal.

In order to facilitate these kinds of results, the facility focuses on a variety of inpatient treatment programs that are designed specifically around the needs of the patients. Much of the primary residential treatment program is built around the 12-Steps of recovery. After a medically monitored detox, if required, the facility's counselors will use a variety of treatment methods to help patients identify issues and learn the coping skills they will need to avoid drinking for the rest of their lives. When apparent emotional or mental problems are creating additional issues, dual diagnosis treatment can be easily incorporated into the treatment process. Shrader has missed drug screens on 4/21, 5/4, 6/23, 8/13, and 8/25/15. On 5/21/15 she was positive for amphetamine. Ms. Shrader was arrested for disorderly conduct on 6/15/15 and signed a statement of admission at the Henagar police department she had been drinking alcohol on 6/12 until 6/13 and used meth on 6/12 to 6/13//15. She tested positive on 8/19/15 for alcohol. Ms. Shrader assessment at the Bridge was scheduled for 4/30/15
The client is a twenty one-year old African American male, unwed and does not have any children and currently resides with his mother was referred to Evolutions Treatment center to address his Substance Abuse Disorder. Fred was born to an unwed couple in Long Island, New York and relocated to Wellington, Florida when he was two-years old due to his mother work schedule. He reported being primarily raised by his maternal grandparents until seventeen-years old, which he later went to live with his mother in Sunrise, Florida. The client described his relationship with his grandparents as "okay" and noted is grandparents, specifically his grandmother was overprotective of him. He noted that he was not allowed to leave the home without an adult supervision.

Mental Status Assessment Appearance Client greeted me at her door with a big anticipating smile on her face. She is a very attractive well groom lady for her age which is 66. She wears black frame glasses that accent her caramel skin tone. She appeared to be very relaxed in her aqua shorts, white sleeveless blouse and house ivory shoes. Her hair was in braids and pulled back in a ponytail.
The client, Chase Robinson, is a 5 year old African American male. The child’s family was referred to CPS after a family friend’s 2 year old son died while being supervised by Chase’s biological mother, Amber Robinson, father, Chance Robinson, and a live-in friend, Anthony Coleman. The case was then transferred to Child Protective Services On-going after Amber, Chance, and Anthony were arrested on first degree murder charges. Chase’s maternal aunt, Octavia Carter, now has temporary custody of Chase and his two younger siblings. Ms. Carter also has 5 children of her own, ranging from ages 5-10.
Basic Information Robbie J, a 19-year-old single Caucasian male. His income and community description is not applicable. He is living with his parents, and is a first-year college student. He has been referred because he has started drinking again and lack motivation. He is currently in family counseling with a social worker on the rehabilitation team.
