Concerns in the area of Emergency Room (ER) physician fatigue are mostly in the areas of patient satisfaction and medical errors, yet many of the concerns regarding quality of care from physicians originate from sleep deprivation. The National Sleep Foundation (NSF) recommends 8 hours of sleep per night for an adult (Malik SW, Kaplan J., 2005). Sleep deprivation can be caused by insufficient sleep or disjointed sleep or both. Adults who get fewer than 5 hours of sleep will show a decline in peak alertness (Carskadon MA, Dement WC, 1982). Fatigue among healthcare professionals is a growing concern. Physicians are held to a standard of safe and proper delivery of care. Physicians are not only expected to deliver effective care but also satisfy patient’s needs. Reducing fatigue may improve patient care and safety as well as improve health care provider’s performance satisfaction and increase communication. Hospitals can lose federal funding for performing poorly on patient satisfaction surveys. Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) is the first national and publicly reported survey of patients perspective of hospital care. This was due to government involvement by Centers for Medicaid and Medicare Services (CMS). The survey results are public so that hospitals will make strides in improving the quality of care. Medical errors In 2015 the federal agency CMS also rolled out a new 5 star hospital rating system. Many hospitals are not happy stating that the methodologies used may not reflect true and meaningful performance. There are over 3,500 hospitals in the rating system and only 251 U.S. hospitals have a 5 star rating. The mission of the U.S. Department of Health and Human Services (HHS) is to “enhance and protect the health and well-being of all Americans” and fulfilling the mission by “providing for effective health and human services and fostering advances in medicine, public health, and social services” (Assistant Secretary for Public Affairs (ASPA), 2016). According to Rouse (2016), the HHS department works approximately one-fourth of federal government disbursements and distributes more grant dollars than all other federal agencies fused together. In order to provide patients to choose the right facility for their health care needs, the Overall Hospital Quality Star Rating can determine where care will be provided. The Overall Hospital Quality Star Rating is designated to assist individuals, family members, and providers to compare hospitals that summarizes existing quality measures based on the patient experience of care data ("First Release of the Overall Hospital Quality Star Rating on Hospital Compare", 2016). There are advantages and disadvantages regarding the Overall Hospital Quality Star Rating.
Students are being woken up in the middle of a deep sleep. Being an alumnus of Melrose High School, I also remember the exhaustion I felt during the week. This can be detrimental to not only the learning but also their health. Sleep deprivation increases risk taking behavior, substance abuse, and impedes judgment and decision making ability. Many districts have taken part in this time shift.
My concept is compassion fatigue. Compassion fatigue (CF) as it relates to nurses working in an emergency department dealing with secondary trauma causing symptoms of compassion fatigue. The measurement tool, which I will use, is the Professional Quality of Life (ProQOL) scale. This scale has been in use since 1995 and has had several revisions, the last one updated in 2010 and it has been translated into 17 languages. The ProQOL measures compassion satisfaction (CS) and CF and its subcategories, burnout (BO) and secondary traumatic stress (STS).

Sleepless in American is a National Geographic documentary on the lack of sleep Americans are receiving each night. The film starts with the statistic that “40% of American adults are sleep deprived” and followed with different effects of sleep deprivation such as: weight gain, delayed reaction time, depression, anxiety, speeds the growth of cancer, and has been linked to Alzheimer’s disease. Although, there is no scientific evidence to support the need for sleep, it is an important process that allows our bodies to function properly. Several sleep studies have been performed to understand the effects sleep deprivation has on a person. The participants of the sleep trial only received four hours of sleep per night.
The statistics don’t lie. Physician burnout is a real problem that exists, and without any type of intervention, can become a personal problem that can ultimately end very badly. No matter if it means that you’ve taken that step to leave the career you’ve been studying for and practicing almost half of your life or falling into an emotional pit so deep that you start filling your life with addictions. In some circumstances, you may be dealing with the beginning of physician burnout without really having a name for what you’re going through. The question that your patients come to you for is how do I deal with their condition, and in this case, you’ve got to answer this very question for yourself.
The patients experience within the hospital is collected from a survey done randomly among patients. Each hospital must have at least 300 survey responses per year. After collecting the data, the data is submitted to the survey data warehouse, where it is analyzed and adjusted to truly reflect the hospital’s conditions. The Centers for Medicare and Medicaid Services along with the Agency for healthcare research standardize the survey results with the hospital consumer assessment of healthcare providers and systems survey. This survey has only thirty-two questions which are analyzed each year.
2 Sleep Deprivation in the Nursing Profession Sleep deprivation is known as a condition of not getting enough sleep that can affect the brain and cognitive functions. "It occurs when a person sleeps for fewer hours than necessary over multiple days or week" (Eanes, 2015). When we think of nurses we are reminded of people who promote the health of others. However, many are unknowingly compromising their own health by working those 12-hour shifts. Many times, its more than 12 hours when they are finally able to go home.

As an aspiring physician in Emergency Medicine, it is hard to describe typical physician workday activities. Every day is unique and filled with many opportunities to learn and develop clinical, interpersonal communication, leadership and critical thinking skills. Despite this, there are certain routine activities which I had the opportunity to observe through my shadowing experience in the Emergency and Operating rooms at California Hospital Medical Center, Los Angeles. One day, a two year old patient came into the ER after falling and cutting her head. The patient’s mother told the physician that her daughter is nervous and scared.
Nurses fatigue is growing problem nurse face each day in the healthcare environment, and he can be caused by long hours, sleep deprivation, and possibly by accepting extra assignments can be dangerous for both nurses and patient. These inadequacies can result in major implications for the health and safety of registered nurses and can compromise patient care which can lead to fatalities. (American Nurses Association, 2014). In my experience, being fatigued from working much 12-hour shifts consecutively was very difficult as I felt extremely tired, resulting in lack of focus, missing important details during the handing over the process with impaired cognitive functioning. This I found was detrimental to the patients and myself as it impedes quality and has a deleterious effect on patient safety.

I’m Nur Atiqah binti maznan and today I will deliver a speech title ‘People should care more about sleep’ Most of us in this class will say that we not get enough sleep because of all assignments, lab reports and so on like just now. So, we called this situation as sleep deprivation which means a condition where people not get enough sleep. We are in the same shoes, so no worries. A research from Brown University stated that, from a survey they conducted to a group of college student, 11% student have a good sleep but the rest which is 73% from the same study were found to have a sleep problem.
The Habits of an Effective Nurse According to Stephen Covey, there are seven things a person must possess in order to be successful. In his novel The Seven Habits of Highly Effective People he addresses just that, ways to achieve success and effectiveness in all aspects of life. Often times, the word habit has a negative connotation; it is defined as a settled or regular tendency or practice, especially one that is hard to give up (Websters, 2014). The key words there are settled, regular, and practiced.
The theoretical framework on fatigue is illustrated in Figure 1. It lays out two effects of fatigue, which may be the positive route or negative route, and is attributed to adaptive and maladaptive coping methods, respectively. The process starts with an assessment by the provider, who then collaborates with other health care professionals for treatment, education and health promotion. However, the individual has free will to follow health promotion or continue to live a stressful lifestyle. The individual is accountable for his or her actions and the concept of nonmaleficence is acted upon if the individual decides with the positive route of health promotion.
