Many psychiatric patients are hospitalized in psychiatric centers, according to the prediction of World Health Organization, their numbers will increase by 15 percent till 2020 [1]. The health and well-being of patients is health care providers’ first priority, and they face challenges to provide quality and safe care in changing conditions, promoting the quality and safety is a constant challenge for them [2-4]. Safety is one of the important areas of health care quality (safe, patient-centered, effective and based on evidence, efficient and fair) [3]. Patient safety is one of the challenging subjects in the area of health care and is very important in different areas of nursing such as educational and clinical [5].

The inductive manifest content analysis process is used to summarize the studies that aim to identify and describe the phenomena. First, interviews were transcribed word by word and studied several times to get an overall sense, then the text was divided into meaning units that were condensed. The condensed meaning units were compared, discussed and labelled with a codes. Then the various codes were compared based on differences and similarities and sorted into thirty four subcategories. Eight categories created from the similarities and differences and seen between the sub categories. Categories were formulated into three main categories (Table 1). In the last phase of the analyzing process, the latent theme was created from a deeper interpretation of the underlying meaning of the subcategories and categories. The data was managed using software

Objective One During my clinical day three, I demonstrated entry-level competence in professional nursing practice in caring for patients with multiple and/or complex unmet human needs. I addressed safety needs, safety in medication administration, effective communication, and surveillance for my patients. First, I addressed safety needs my ensuring the appropriate safety measures were implemented for the patients. Some of the safety measures included, wearing non-skid socks, wearing a yellow armband which indicated fall risk, keeping the bed in lowest position, two side rails up, bed locked, and the call light within reach.
This means that not only must the patients learn to be “normal” but the staff must learn to be “insane”. The hospital staff must know and realize and take into account the cultural reasons and other aspects of the patients to be able to care for them, which helps in the recovery and treatment process of one day conditionally releasing the
Offenders with these disorders could be dangerous to other or even themselves. The offenders with disorders like those stated before are close to being released back into society which must be frightening if they do not take their medicines. 2 Improve conditions Mental offenders are thrown in a correctional facilities due their crimes, but these offenders should be in a prison or a correctional facility where they do not get the help they need. When in a prison some of these offender could be and will be picked on like a schoolyard which might led to a suicide attempt. A report on mental offender in california have reported that over 30,000 prisoners confined in the state prison.
Many inmates in the correctional system have been incarcerated with a mental illness or has developed it while being in custody. Even though they place them in a separate section they are still in serious danger of harming themselves or having others harm them. Solitary confinement has also played a key role in those who develop mental illnesses. Many have suggested that solitary confinement is a serious threat to many inmates.
These inmates have also reported feelings of panic and rage, that they had poor impulse control during their stay in solitary confinement. Compared to the general prison population, the rates of suicide and self-harm as particularly high in the isolation unit. Both can be a result of depression, or poor impulse control. More psychological symptoms that prisoners may experience disrupted thinking, hallucinations, and illusions. In extreme cases inmates get to the point that they exhibit full-blown psychosis and require hospitalization to be helped and treated.
Tuesday, March 29th, 2016: Montana State Hospital The day began with a care conference by an interdisciplinary team (psychologists, social worker, nurses, and psych techs). Every patient on the wing was discussed about their current situation, behavioral problems, or change in care plan. The nursing process seemed to be utilized when dealing with these problems. The problem was assessed by the entire team, the root cause was determined, a plan was established to implement, and then would be evaluated the next day.
Patient safety experts have demonstrated that “patient safety increases when teamwork and collaboration skills are taught and empowered; when teamwork and collaboration are not present, medical errors will result” (Creasia & Friberg, 201, p. 348). As a nurse, it is imperative to collaborate with other interdisciplinary members in health care and also strive to research and implement evidence-based practices. Evidence-based practice is necessary to “ensure the highest quality of cost-effective care and the best patient outcomes” (Fineout-Overholt, 2011, para. 16). With a collaborative and innovative attitude on safe health care practices, an increase in patient safety and effectiveness of care will
Web. 15 Oct. 2015. Recently, the issue of treating mentally ill inmates while they serve time for the crimes they committed has become a very prevalent topic (Glazer n.pag.). Because budget cuts have caused many mental health institutions to shut down, court and law officials have been led to place these mentally ill offenders in jails that do not have the equipment and staff necessary to help treat them (Glazer n.pag.). Instead, the mentally ill offenders are simply placed in solitary confinement, causing their condition or illness to worsen over time (Glazer n.pag.).
Mentally Ill in the Criminal Justice System The warehousing of mentally ill patients with in the justice system has recently began to receive a great deal of attention from concerned media centers and citizens of the aforementioned justice system. As many people know, there are a variety of different types of personalities within any penitentiary or prison. The warehousing of mentally ill patients just contributes to the list more. It has become more inhumane to house the mentally ill patients throughout the criminal justice system due to solitary confinement (Reutter).
There are so many mentally ill people in correctional facilities because most families do not know how to help their loves ones who suffer from a mental illness, so the call the police for help. Majority of the police officers do not know what to do or how to handle people with a mental illness disease. Police officers who are not trained to deal with the mentally ill often do not recognize that person is ill. Some police officers do not recognize if the individual should or not go to jail or a treatment center or medical facility. The impact of law enforcement and the judicial system dealing with people with a mental illness is to assist the inmates with the help they need.
However, there aren’t many options when it comes to treatments for mentally ill patients, and often times, the wrong treatment is elected. After mental institutions are adequately funded, the next step is to safely transfer those who are mentally ill found in jails and hospitals to these new sanctuaries. Studies have shown that, “Alternative-to-incarceration program for adults with serious mental illnesses are also very important. Individuals who go through them, on average, spend less time in jail and get more comprehensive treatment.” (Louison.)
Methodology In my thesis I utilized the quantitative content analysis method. There are many definitions of this research method from different authors. Most of the definitions include similar principles. The research should be systematical, objective, quantitative and replicable.
