SC attempted to explain why the service was interrupted and next step; SC end call with Marguerite. SC then called Vital Support and briefly explained to receptionist and requested to speak with a manager. SC was transferred to Vitaliya at Vital Support Nurse manger. Vitaliya stated that the termination date is incorrect and service was provided to Pa beyond termination date and that SC need to change the dates so that they can receive payment.

Individualised approach to care planning essay The care planning process is a fundamental part of nursing, Barrett et al (2012) emphasises the importance of the process by recognising it as a clinical skill that needs to be learnt and developed. Passion 4 Community’s support, through both psychosocial counselling and facilitating enrolment in income generating activities, has proved successful in addressing the particular needs of the youths suffering from extreme poverty as a result of the conflict. A recent example of this can be illustrated by the assistance provided to a young girl aged 14 who was struggling to take care of her 5 siblings as a result of the loss of her parents, who both died of alcoholism following the conflict.
