In summary, on 09/25/15 at 0159 hours I was patrolling the area of 1800 S. 59th Ct., when I observed a vehicle (02 ' Pontiac IL V707523) traveling southbound at a higher rate of speed than the posted speed limit (20mph). I paced the vehicle at 35mph for approximately for two (2) blocks (1900-2100 59th Ct.). The vehicle had an obstructed front windshield. I made contact with the driver at which time I requested a driver 's license and insurance for the vehicle. The driver was unable to provide any form of identification, but was able to provide insurance. The driver related he lost his identification and has not replaced it. I observed two (2) clues in total. Sanabria was placed into custody at 0216 hours and he was transported to the Cicero Police Lock-up to be processed. I conducted a search of the vehicle for any signs of alcohol use and observed a can of Monaco alcoholic beverage underneath the passenger seat. Two (2) photos were taken on the scene and later placed into inventory. The vehicle was towed by Tuff Towing (15-3177) and D.U.I. impound packet was completed on the vehicle. I read Carter the Warning to Motorist from a pre-printed form at 0159 hours. He agreed to perform a breath test administered by Ofc. E. Lara #244 at 0247 hours, who is a State of IL certified breath operator. The test results were (.090). I read Sanabria his Miranda warning from a pre-printed form at 0248 hours and he agreed to speak (see Alcohol Influence Report). While I spoke with Sanabria I detected a strong odor of an alcoholic beverage.
Sanabria was charged with above listed offenses and he was advised of a Maybrook Court date (10/13/15 at 1:30 P.M. room
On 11/07/2017, the agent received a Law Enforcement Contact form indicating offender Joe A. Slugger was at the K&D Tap on 11/06/2017 having unauthorized contact with Sandra Williams. It was stated in the LEC form that Joe Slugger was drinking alcohol and will be referred for the charge of felony Substantial Battery by Badger Police Department after he was witnessed to hit Sandra Williams with his open right hand to Sandra William’s left side of her face and then grab Sandra Williams by the back of her head and slamming her head onto the bar counter where she was seated. Sandra Williams was observed having a visible red mark on her face and a laceration on her forehead that later required eight stitches. On 11/08/2017, the agent received a police report from Badger Police Department Officer Konkol (Case number 1-98-00456723) stating Officer Konkol and Officer Sandner were dispatched to the K&D Tap (223 W. main Street, Badger WI) where they made contact with Joe Slugger.
On 10-03-2015 at 2018 hours I responded to 599 Pine (Casey 's) in reference to counterfeit bills. Upon arriving I made contact with Store Manager Laura Isaac and Officer Woodruff. While Isaac was pulling up surveillance footage she explained that a female had attempted to pay for gas and other items with a counterfeit twenty dollar bill. I seized the note, it has been entered into evidence as #18596. Isaac also explained that the female arrived in a vehicle which she described as a grey short cab dodge truck.
On 01/23/2016, at approximately 1428 hours, your affiant was on routine patrol travelling north on State Route 924 (North Main Street). Your affiant observed a black Subaru station wagon travelling south on State Route 924 approaching a steady red traffic signal located at the intersection of Main and Coal Streets. The operator of the Subaru continued through the intersection and travelled through the steady red light. Your affiant activated the emergency lights and conducted a vehicle stop on the unit block of North Main Street. Your affiant approached the operator and requested his license and vehicle information.
Alice Odom walked into the Gautier Police Department to report that someone had rifled through her vehicle, 1997 Mercedes and took $50.00. Odom said that there were four vehicles parked in the yard of her residence at 3140 Breakwater Dr. All four of the vehicles were rifled through, but Odom only had money taken from one of the vehicles. The incident happened between 8:00 p.m on 10/03/2016 and 5:15 a.m. on 10/04/2016.
P.O. Carlus Ingram and P.P.O Harris responded to Interstate 70 Westbound exit ramp at Salisbury Street for a call for a “Department Accident.” After responding they were advised by Driver 2 that he was stationary on Interstate 70 westbound at Walnut Street relative to a call for construction debris on the highway. Driver 1 was traveling westbound on Interstate 70 approaching the rear of Driver 2 patrol vehicle. While Driver 1 was attempting to merge into another lane he side swiped the diver’s mirror of Driver 2 vehicle. All parties stated they were not

On September 6, 2015 at approximately 8:21pm I, Deputy Tucker George, checked en route to the Shell Gas Station in Hawkins, Texas 75765, Located at 102 East front street in reference to an assault call. Upon arrival I located the complainant Michele Turner inside the Shell station sitting at a small table on the North wall of the building just inside the door. Michele Turner appeared to be intoxicated upon my arrival and displayed multiple signs of intoxication throughout the process of conducting my investigation. Michele Turner was unable to provide detailed information in regards to her assault, but did state the house at which she had been and was assaulted was a known dope house. On August 22nd, 2009, the police pulled David Leon Riley over for driving with expired license registration tags. Due to Riley’s license being suspended, the policy, through their policy, required his car to be impounded. In addition to impounding the car, they are also required to conduct an inventory search to ensure that the vehicle had all of its components at the time of the seizure, to defend against future liability claims. It also serves as a way to search for hidden contraband. During the search of Riley’s car, the police found two guns, arresting Riley for possession of firearms.

Both parties agree that Aguiar asked Flores several times to remove the handcuffs which she did. However, Aguiar claims that Flores told him that she was arresting Mrs. Vazquez for drunk in public while Flores claims that she placed handcuffs on her for her own safety. Aguiar also claims that Flores was rude when she threw her hands up in the air as if frustrated, saying, “Fine, whatever,” while walking away.

At point A corner of 74th St and Madison Avenue a LTB Mechanical Inc. employee parks his Van and seems very happy ( my personal comments S.H.) after find parking because of busy traffic in city. The driver goes to the nearby muni meter and inserts his credit card and he presses couple of buttons but was unable to buy the time card. He goes to the opposite corner and disappears.
Facts On Friday, September 4, 2015, approximately at 3:00 PM, Dr. Melanie Nolan-Pryor (46) and her husband, Dr. Brent Pryor (46), left a luncheon in Sacramento in separate cars. Dr. Nolan-Pryor was driving her blue 2013 Lexus SUV, while Dr. Pryor was in his silver 2014 Toyota Prius. Traffic on I-80 was slow; Dr. Nolan-Pryor estimated her speed range from 20 to 40 miles per hour. As the couple approached the Mace Boulevard exit, Dr. Nolan-Pryor’s car was located “in the lane to the left of the right-hand lane”, and Dr. Pryor was traveling in about 10 car lengths from her in the lane to her right, with a black Toyota Tundra truck in front of him and a red Ford Escape SUV behind him.
