In this essay I will present and analyse my personal development and achievements along with the challenges I was faced with, in the four domains of clinical care, that were the result of applying the theoretical knowledge and practical skills acquired in the last two years of this course into clinical practice. Additionally, I will be providing an example for each of the four domains in the appendix section, in order to better demonstrate these points. Finally, I will conclude this essay with a reflection of my own learning outcomes as well as a recognition of any and all areas that I find challenging and myself in need of further development. I had to learn of the potential sypmtoms and remain constantly aware of them during my interaction with the patients as I tried to build a therapeutic relationship with them around those issues. Moreover, as not all patients experienced the same symptoms, a different approach was needed in each case. An issue that I found particularly challenging was that some of the patients did not want to reason with the fact that the hallucinations/delusions they were experiencing were not real and even when they came to accept them as false they did not always want to silence them or make them disappear (see appendix A). Therefore, in accordance to the NMC code (2008), I had to learn to repsect each person's decision about their treatment and encourage them to work together with me in order to develop a care plan that better suited their individual needs and wishes. Thus, it became evident to me that using a person centred approach n my practice was crucial in order not only to achieve the best possible therapeutic results, but to also empower the patients through the process . Person centred practice is where all service users should be treated as individuals and their care that they require and support needs should reflect this. Each and every service user will have specific individual needs and their care plans and support should be tailored to suit their needs. By seeing the person as an individual and recognising their diversity puts the individual at the centre of their care. Person centred practice is not only about supporting people with their individual support needs and care but also about getting to know the person, what their likes and dislikes are, what makes them happy and bring them joy, knowing what their values are, family situations, social circumstances and lifestyles.

Reflecting on the past seven weeks I have acquired countless knowledge, which I will use to further strengthen my profession as an Advanced Practice Nurse. The course allowed me to think beyond my current practice knowledge and acquired innovative ways to evaluate the situation at hand. The learning objective in program outcome four helps set standards that I will use to guide my clinical practice to meet various healthcare needs. Using the case studies has helped to further enhance my knowledge on disease physiological state, using differential diagnosis, disease manifestations, and clinical presentation. It has also taught me the skills on how to differentiate between similar diagnoses to properly identify the problem and treat the patients.
After reflecting on all that you have learned in this course, share what you believe is one current trend in the human services field in meeting the needs of individuals, families, and/or the community. This course has been a revelation for me. In the past eight weeks, I learned many things about running a nonprofit organization, and I am sure I will learn a lot more. There are so many different elements that goes into managing an organization like how to build capacity, funding/budgeting, assessment tools, the role of the board of directors, and past/current trends in the field.

Person centred thinking is when you put the thoughts of the person you are looking after before your own. It’s important to know how they think and feel to know what to put into their care plans so that they are supported in the best way possible and to make them feel included 1.2 Explain the benefits of using person-centred thinking with individuals? By using person centred thinking you know how the client feels and how its best to support them but you also know what goals are possible to set for the future and also any changes that need to be made.
This week I met with a social worker at Cherry Health which is located a few blocks from Heart of the City. When I met with the social worker we did a role playing exercise. She played a client that was experiencing depression and sadness. In addition, she role played as a client that she typically work with at the agency and I role played as the therapist. I enjoyed the role play because I was able to receive feedback about my progress.
As its evident that person centeredness is valued by the person, encouraging it in practise is important. The aim of the framework of McCormack’s model of person centred care is to raise awareness of the importance of respect for the individual. It does this well by empowering healthcare workers to recognise key components in their practise. This essay will discuss the positive impact that person-centred care can have on staff and residents in long-term care settings, using the example of Seven Oaks care home. Firstly this essay will define the key terms of person-centred care and define the meaning of long-term care settings. It will then look at examples of the positive impact of person-centred care for both residents and staff in the example of Seven Oaks dementia care unit and the case study of Rita Wallace, which demonstrates the individuality of person-centred care. What is reflection? Reflection is described as the process individuals use for self-development in their future career. The process of reflection has been used for many years in professional health fields such as midwifery and nursing (Lillyman. S & Merrix. P, 2012). Florence nightingale pioneered the practice known as reflection-on-practice, this is a tool that is needed in developing improvement and knowledge to enable an individual to grow in their nursing profession.
It enables the student to reflect and consider his/her intentions and actions prior to the incident and prompts the health professional to critically assess the incident and identify the key learning outcomes through a series of questions (Johns, 2013). These two models can be interlinked to enable the student to explore his/her thoughts are feelings without making assumptions whilst creating learning opportunities to change future nursing practice. Due to having two different models of reflection, where Johns (2000) reflective model does not permit the practitioner to delve into their thoughts and feelings, Driscoll (2000) model of reflection enables them to do so to achieve different learning outcomes (Jasper, 2013).
Person-Centred Care aims to ensure that the older adult is an equal partner in their health care. Key components that ensure PCC is provided are the following: respect and holism power and empowerment choice and autonomy empathy and compassion. (Rcn.org.uk, 2015) A person-centred approach to nursing focuses on the individuals needs, wants, goals and desires so that they become central to the care and nursing process (OpenLearn, 2015). According to The Department of Health (State of Victoria, Australia), person-centred care is a philosophical approach to care, ensuring that service systems are developed in partnership with older people and/or their carers (Health.vic.gov.au, 2015).
Person-centred nursing is widely practised in clinical areas today, the original concept was developed from the work of psychologists such as Carl Rogers and Tom Kitwood. Rogers (1957.1961) considered empathy and unconditional positive regard to be core features of any therapeutic relationship in counselling. He developed the concept of person-centred therapy in counselling. Stein-Parbury (2009) writes about the use of interpersonal skills in nursing and places a focus on Roger’s model of person-centred therapy. She states that person-centred nursing models have been influenced by the work of Rogers.
The Gibbs’s reflective cycle comprises of six stages, which are description, feeling, evaluation, analysis of the incident, conclusion and an action plan. I used these stages as a guidance tool during the process of reflective essay about my critical incident (Parsons and White 2008). Nursing in the past was more habituated and ritualistic than rational and precision. Nurses were not encouraged to question their practice let alone reflective practice.
