Evidence-Based Clinical Care: A Case Study

My knowledge in the Doctor of Nursing Practice (DNP) continues to expand as I progress through the DNP program. The knowledge I have attained while in the study of the DNP I and DNP II is priceless. For example, recognizing stakeholders and utilizing networking has helped me to progress on my DNP project (Gokenbach, 2012). I also have a better understanding as well as the purpose of a DNP project. My mentors have been great in directing me in remaining on the right track.

In 2010, President Barrack Obama signed the health care reform into law, the Affordable Care Act (ACA). Community-Based Collaborative Care Network Program is a part of the ACA which emphasizes primary care, funds community health initiatives, and promotes quality care for more coverage for those uninsured in the U.S. and is more cost-effective to generate billions of dollars in savings for the United States budget. Working with state office leadership to develop policies and procedures based on evidenced based practices is crucial to the health of the public. The director attends county board of health meetings to professionally represent nursing and converse with board members to identify interventions to assist with addressing the needs of their community. These responsibilities highlight the need for all public health nurses, especially those in leadership roles, to have a BSN degree. As pointed out, the opportunities for a nurse transitioning from an AD position to a BSN position are diverse and endless. Patients, healthcare facilities and the nursing profession benefit from advanced degree nurses.

It is estimated by 2020 there will be a shortage of 60,000 RNs in Canada (McGillis, Gates, Peterson, Jones & Pink, 2014, p, 54), a fact that is exacerbated by the implementation of a completely foreign examination system. The baby boomer generation is entering a stage that requires additional healthcare resources, a reality that is supported by the fact that the geriatric population is expected to increase up to 22.8% by 2031 (McGillis et al., 2014, p.54). The projected increase in geriatric population is proportionately linked with the increased demands of healthcare services directly translating increased pressure on the health care system. The need for evidence based practice will help ensure the patient will receive the highest level of quality of care. Each member of the health care team, work together with the common goal of identifying the needs of the patient and offering the most safe, efficient, and effective care. Patient and family centeredness is one of the values identified with the decisions that are being made on the behalf of the patient with evidence based practice (Melnyk and Fineout-Overholt, 2015, p. 176). Evidence-based practice refers to problem-solving and making the best decision to solve problems with evidence produce in healthcare settings. Numerous researched study on evidence-based practice shows that, it is very useful for healthcare providers to identify the needs in patients and providing the relevant interventions which helps to improve the care process towards patients during hospitalization. My team members and myself decided to focus in the topic in the most effective exercise programs which can reduce falls among the older adults in hospitals.

The DNP student implied that this program was funded by the Florida Blue Foundation. The DNP student discussed the process and challenges endured during the program. The outcome of being a participant in this program was the opportunity to connect with other nurse leaders, and learn the process of policy development build on leadership skills. Several individuals in the audience wanted to know more in details about the student’s DNP project.

The Affordable Care Act has shifted focus on health and wellness of patient populations urging hospitals to do a community needs assessment and come up with a strategy to address these needs. Hospitals most often partner with community and professional organizations to address the health need (Stempniak, 2014). This is an example of how the four spheres are all interrelated. I care about providing the best holistic healthcare I possibly can for my patients through evidence-based practice. I respect individuals’ race, socioeconomic class, gender, and sexual orientation. I work well with others and I strive to help create a good working environment where people can communicate, ask questions, learn new information, and feel respected. With this New Graduate Nurse Residency Program, I hope to be exposed to many different clinical situations. I hope to further develop my clinical leadership skills and to gain new knowledge.

As there continues to be an initiative to improve quality healthcare while simultaneously reducing costs, the importance of the nurse with advanced education in transforming healthcare delivery and healthcare policy will continue to grow. Savor reviewed research that interviewed nurses and explained how many nurses “pointed out that the demand for nurses is rising, with only slow increases in supply, and that nurses’ earnings flattened” (2017). This can lead to a shortage of nurses because in current day society, living has become much more expensive. For example the cost of housing has increased dramatically, and students who go to college are now in charge of paying back their tremendous amounts of student’s loans. All these expenses add up and can be discouraging to a student trying to enter the nursing field because starting pay of a nurse is not enough to live comfortably in current day society. Governments are beginning to realize this and are seeing the decrease of nurses entering the field.
