If current rates continue, by 2030 this number will rise to about 3 million (CDC, 2014). Nursing home residents are at an increases risk for falls depending on the acuity of their illness and their mental state.

Evidence- Based Practice Evidence based practice has influenced the way that induvials view sexually transmitted disease. It allows us to look at the information we have gathered previously, interpret it and then determine what needs to be changed and what seems to be working properly. When looking at EBP, we can tell that with the use of protection such as condoms and dental dams, the percentage of STDs being transmitted has lowered. When sexually active individuals did not use preventative measure, the percentage of STDs grew. EBP also has proven to show that the more partners one has, the risk of catching an STD becomes higher.

Falls account for a majority of the injuries to patients in long term care facilities. With falls being such a prevalent problem in the geriatric population it is important for nurses to take into account many different aspects of the problem and make it a priority to reduce and even eliminate the problematic issue with the long term care setting. The project studied in this article used evidence-based practice TeamSTEPPS®, which stands for Team strategies and tools to enhance performance and patient safety. TeamSTEPPS® was used to improve teamwork and communication within the hospital setting. TeamSTEPPS® provided the nursing staff with a structured, yet simple way, to identify fall risk factors for each patient and patient-specific interventions. Problem Identification Getting out of bed is one of the dangerous things that the elderly patients do when they are admitted in the hospital. Study conducted by Ambrose, Paul & Hausdorff, (2013) on patient falls reveals that a majority of falls in the elderly patients occur between 0700 and 1900, especially when they are getting out of bed to use the rest room. The cause of their falls is mainly due to unsteady gait, memory loss, confusion that comes with age. Memory loss and vision problems which occurs during old age in the elderly patients puts them at risk for falls. Other factors that can lead to falls are; Presence of throw rugs, psychotropic medications, lack of Vitamin D, and weakness of the lower extremities.

The Impacts of Evidence-Based Practice on Service Provision for Children Placed in Foster Care Children in the United States who have been abused or neglected by their parents or caregivers are commonly put into the foster care system, a system designed, at its best, to provide a safe alternative to the child’s home while legal decisions or made or during attempts at family reunification. In many cases, children in the foster care system have come from homes in which child abuse and neglect are prevalent and may have experienced significant adverse events, including substance abuse, childhood sexual abuse, domestic violence, and physical violence. Unfortunately, the foster care system is an imperfect arrangement.

The management had sense of urgency to implement change in order to decrease the percentage of patient fall in the hospital. As cited by Bradley (2016), there were necessary to change to develops in ordered to meet the goal. Minority of the nurses showed a positive respond in changes because they believe changes helps to improve their knowledge and provide better nursing care in preventing patient fall. However, they were majority of senior staff nurses showed negative respond were against the changes.

It also provided the use of critical thinking and clinical judgment on how to prevent falls, support, and be accountable for a client professionally. The practical knowledge I have learned helped me become aware of assessing and assisting a client. As a nurse, our job is to provide “safe, compassionate, competent and ethical care” (p.8) and collaborate as an interprofessional team to deliver safe care and prevent risks from happening while offering quality nursing care (CNA, 2017). I will always provide the professional care under the code of ethics to promote health and wellness for an older adult and prevent risks from happening. As well as following the plan of care, use communication strategies, be aware, acknowledge, and accommodate individuals with different diseases such as with dementia, to promote fall prevention strategies (RNAO, 2017).

INTRODUCTION: This contextual project consists of 10 concept from the block 1 study with the title “Research and Nursing Research”. The meaning of each concept will be clarified, critically analyzed and applied to social context, personal life and current professional life. The usefulness of the concept to the current world will also be explained.

Wendy, I agree with you that not only DIKW hierarchy is good for evidence-based practice but most importantly is communicating to our patients. As a nurse, we are not only providing safe care for our patients but educate them through our knowledge and wisdom that we learned from the information through the classes. We communicate with them to prevent further complication or simply help them to maintain good health. DIKW hierarchy definitely is not only for nursing practice but good for our daily lives. Every day we are learning something new either from our practices or simply watching the news.

This program, called the Nijmegen Falls Prevention Program, included one hundred thirteen elderly clients with a history of falls. Exercise sessions were held twice a week for five weeks with fall monitoring done before and after the experiment. Control assessments were also done continuously thru the study to determine client changes in standing balance, balance confidence, and obstacle avoidance skills. The results of the Nijmegen Falls Prevention Program showed that the number of falls within the exercise group dropped by a significant forty six percent! Not only less falls, but obstacle avoidance skills dramatically improved as did balance

Chapter 5 Discussion 5. Discussion This trial was conducted to find out the efficacy of intensive strength training protocol over conventional physiotherapy treatment on balance, gait, and mobility in post stroke population. Totally 60 participants recruited from the population and randomly allotted into experimental and control group. The experimental group received intensive strength training to lower extremity, upper extremity and trunk muscles for 6 weeks, 6 days per weeks and 2-3 hours per day. The control group treated with conventional Physiotherapy exercises for the same duration.

Evidence-based practice is an all-around systematic approach to patient care that was built up on research and proven treatment results within nursing in order to increase the patient outcomes. Evidence based practice is define as “the integration of current evidences and practices to make decisions about patient care”. (Medical Surgical Nursing, 2018). Evidence-based practice not only includes the best proven research for practicing patient-centered care, but also merges the patient’s preferences and values into consideration.
