During Q1-Q2 2015, I have achieve 5 out of 6 KPI target. The numbers provided below are average KPI from January to June 2015. Quantity of achievements: • Percentage of invoice posted against PO is 90.45%, which the target is 81%. Many people are supported in their homes by people who work along side the health and social care service such as social workers,nursers ,doctor and care givers or they may get support from their family members and friends. An elderly person may choose to stay in their own homes instead of a residential care home while seeking help and support from others,however this can lead to potential opportunities for abuse happening to the person who Is being looked after. When an individual is getting support at home by a carer, the individual being cared for is vulnerable and defenseless and therefor this makes them a target for abuse,another example is abuse by strangers,when an elderly person lives alone they are at risk of people calling at the door and then gaining access to their homes,they may steal items from them or mistreat them or cause intimidation. A person who is vulnerable and living alone are at risk of physical and sexual because there is no one there to stop this abuse from happening.

In mental health there are a lot of different roles of different agencies and professionals within the assessments, both of these are for the protection of the service user and service provisions these include mental health services which are set up and run by the NHS organisations as well as some voluntary sectors there are also private companies which are paid for by or commissioned by the NHS. A local GP should be able to recognise a service users symptoms and diagnose the service users symptoms of mental health or psychosis then they should refer the service user to an intervention service were they will then be assessed. I have different types personal values and principles including but not limited to; honesty, polite behavior, gentility, maintaining my dignity, creativity, courage, tradition, religion, freedom and liberty. Also, there are some principles attached to the health and social care workplace these principles include work values which are formed with the help of teamwork, dedication, competitiveness, effective communication, trustworthiness, reliability, professionalism and ethics.

7 / D.P7: Explain how different procedures maintain health and safety in a selected health or social care setting Maintaining health and safety in health and social care is extremely important to ensure the health, safety and wellbeing of all their service users as well as other individuals service providers may come in contact with in the setting. There are several procedures that help to maintain this health and safety however they can all vary between settings for example, health and safety procedures will be slightly different and more focused on certain areas in hospitals and especially in paediatric ward compared to in drop-in centres where the needs and risk to service users are slightly different. Some of the procedures used in health and social care to maintain health and safety include; infection control and prevention, safe moving and handling of equipment and individuals, food preparation and storage, storage and administration of medication and storage and disposal of hazardous substances.

Health Assessment A comprehensive family assessment provides the foundation to promote family health (Kaakinen, 2010). Gordon’s functional health assessment is a holistic approach describing the individual’s biopsychosocial-spiritual, interrelated behaviors in 11 different areas in a family/community life (Edelman & Mandle, 2010). The author of this report, assessed a family of four that live in Aurora, Illinois by asking open ended questions in the eleven categories of Gordon’s functional health pattern theory. The author then analyzed the data and developed a wellness diagnosis for the family.

A Community Health Needs Health (CHNA) is a process designed to identify health issues within a community, recognize the neediest, ensure the accessibility of health care resources, and ensure the good use of those resources. The ultimate goal of a community health needs assessment is to promote community healthcare by identifying and prioritizing health needs, knowing the available resources, and proposing an adequate action plan to address needs (Pennel, McLeroy, Burdine, Matarrita-Cascante, 2015). However, the assessment method is different form one researcher to another. In fact the best community health needs assessment approach remains unknown (Becker, 2015). However, there are many critics among the scientific community on the quality of some approaches.
